Your enrollment period is an important time to review your benefit options and choose the best coverage for you and your family. You can enroll for benefits within 60 days of eligibility or during the Open Enrollment period. No enrollments or changes will be allowed after your initial enrollment period or after Open Enrollment ends, unless you experience a qualifying event (see Enrolling & Making Changes for details).

Enrollment Is Easy

• Visit big5employeeportal.com and log in to enroll.
• Click the Myself icon.
• Choose Benefits from the drop-down menu.
• Select Manage My Benefits.
• Submit your elections.
• If applicable, submit all required dependent verification forms to the Benefits Department at benefits@big5corp.com. If enrolling online, dependent forms will be uploaded directly to the site.
